Health Information Forms

Chapter ATCP § 78

Any camper spending an overnight at Boston School Forest is required to bring 2 copies of the Health Information Form. The health information form is only good for the designated overnight dates. There are two forms that have approved by the state. 

BSF Health Information Form - Available for any youth or adult

Annual Health and Medical Record Form - Available only for Scouts
