Printable applications are available for download in English, Spanish and Hmong.

Please take care to fill out the application completely and accurately. Inaccuracies and missing information may lead to a delay in the processing of the application.

Electronic applications are available through Skyward Family Access. For instructions on navigating to the electronic application, please see our FAQ page.

To submit your completed paper application, you may choose one of the following:

  1.   Leave it with the secretary at the main office of your student's school
  2.   Drop it off at the Food Service Office (3400 Water Street, Stevens Point)
  3.   Mail it to the Food Service Office
  4.   Email it to the Food Service Office

Fee Waiver Request Forms

Please fill out the fee waiver request form completely and accurately before submitting it to the main office of your child's school. Fee waiver forms are not available electronically.
